All Athletes must have a signed, valid LPS Sports Physical form Download LPS Sports Physical on file with the athletic director no later than the date indicated on the sign up form for the sport they are trying out for.

Please print both pages and read the entire form, and be sure to take this form to the doctor to fill out medical information and sign and date.

The form WILL NOT be valid without a doctors signature and appropriate date.  To be valid for the 2022-2023 school year, a sport physical must be dated AFTER APRIL 15, 2022.
